If your medical professional does not have a ZERAMAX™ access code, he/she can obtain one by calling 888-ZERAMAX (937-2629) and speaking with a customer service representative.
5. Why do I need my medical professional’s ZERAMAX™ access code?
You need your medical professional’s ZERAMAX™ access code in order to purchase ZERAMAX™ because it ensures that your use of ZERAMAX™ was approved by a medical professional.
6. Where can I find my medical professional’s ZERAMAX™ access code?
Your medical professional may have given you a piece of paper with the ZERAMAX™ website address and his/her specific ZERAMAX™ access code. In addition, if you received a promotional sample of ZERAMAX™, it is likely that the product came with a card that specifies your medical professional’s ZERAMAX™ access code. If you do not have your medical professional’s access code or cannot find it, please call customer service at 888-ZERAMAX (937-2629) and one of the customer service representatives may be able to assist you.
7. When I purchase ZERAMAX™, what will be the charge on my credit card?
If you choose to purchase a single bottle of ZERAMAX™, you will be charged $52.99 per bottle plus shipping charges on your credit card. If you choose to participate in the ZERAMAX™ Advantage Program, you will be charged $38.99 each month until you cancel your participation. After the first three months of participation in the ZERAMAX™ Advantage Program, you may cancel your enrollment at any time if you no longer wish to continue to participate. No further charges will be made after you cancel your participation in the ZERAMAX™ Advantage Program. Charges will appear on your statement as JDS Therapeutics.
8. How long will it take to receive my order?
You should receive your order in 5-7 business days with regular shipping.
9. Do you offer expedited shipping?
Yes. We offer regular shipping services for $4.99 and USPS Priority Mail shipping for $9.99. If you participate in the ZERAMAX™ Advantage Program, you will not be charged for shipping. By enrolling in the ZERAMAX™ Advantage Program, you should automatically receive your new shipments each month before your previous month’s supply of ZERAMAX™ runs out.
10. How do I track my shipment?

11. What is the return policy?
The makers of ZERAMAX™ guarantee your satisfaction and will accept returns of unopened bottles under the conditions stated below:
Individual Bottles: For returns of individual unopened bottles, please call customer service 888-ZERAMAX (937-2629) to obtain a return authorization code and instructions. Customers are responsible for return shipping costs and only sealed and unopened bottles postmarked within 30 days of purchase will be refunded.
ZERAMAX™ Advantage Program: You may cancel participation in the ZERAMAX™ Advantage Program any time after three months by calling customer service (888-937-2629). No returns are necessary and no further charges will be made after you cancel.
Damaged or Defective Product: If you believe you have received a damaged or defective shipment, please call customer service within 10 days to receive a replacement free of charge.
